Training Course Registration Form. You may register online or by mail.
To register for a course, complete the registration and submit payment (check or purchase order) to PQNDE Services at least two (2) weeks prior to the first day of the scheduled course date. Other arrangements may be made in advance when approved in writing by PQNDE Services
Fees include textbooks, handout materials, training quizzes, final written general examination, Continuing Education Units (CEU) registration and course certificate.
A course registration may be rescheduled, canceled or postponed without penalty to the registrant when written notice of such request for rescheduling, cancellation, or postponement is received by PQNDE Services at least seven (7) days prior to the first day of the registered course schedule date. Otherwise, only 50% of the course fee will be credited or refunded.
In the event PQNDE Services reschedules, cancels or postpones a course, a minimum of seven (7) days notification will be provided and a full refund or credit will be issued.
